Comparison of Modified Cronin¢¥s Method and Wardill¢¥s Method in Palatoplasty

Abstract
There are many surgical procedures in cleft palate repairs, but no satisfactory postoperative improvement was obtained in speech, the most important goal of repair.
Cleft palate patients have some anatomical abnormalities in the palate; shortpalate, hypoplastic velar muscles and their abnormal insertions. In order to achieve effective velopharyngeal closure for speech, these abnormalities were compensated with palatal lengthening techniques.
We used nasal side mucoperiosteal flap, the modified Cronin¢¥s method, in pushback palatoplasty for more posterior advancement of soft palate rather than usual Wardill¢¥s pushback palatoplasty. The raw area on the palatal bone and soft palate was gradually healed with granulation tissues. Just after operation, a marked palatal lengthening was obtained by using modified Cronin¢¥s method rather than Wardill¢¥s pushback palatoplasty but the lengthening did not maintain during the period from 5 to 24 months of postoperative follow up.
In lateral static pharyngography and fiberophtic nasopharyngoscopy, there was no significant difference on palatal lengthening and postoperative speech status in both groups processed with modified Cronin¢¥s method and Wardill¢¥s pushback method.
It is concluded that the result of modified Cronin¢¥s method was possibly due to scar contracture on palate during granulation healing process of raw area on the nasal side mucosa.
